What Is Fertility Awareness? A Beginner’s Guide

Let me guess: maybe you got here because you found out that women are not fertile every day of the month (shock!), or because you are tired of not feeling like yourself under hormonal contraceptives and you’re desperately looking for an alternative? Maybe you’ve been trying to conceive and you realize it’s not so easy for everybody, and you want to treat the root cause of your fertility issues to be a vibrant and healthy mother to your child? Or are you struggling with PCOS, irregular or absent periods, period pain, excessive bleeding, severe PMS and you know there must be a way to heal naturally?

Well, I have good news! There is a whole world of fertility awareness waiting for you. This is a science-based practice, 99.4% effective for contraception (in the method that I teach), and can definitely help you with your health and fertility issues.

Are you ready to dive in? Here we go!

Fertility awareness is the practice of tracking biological signs of when you’re fertile (able to get pregnant) and when you’re not. That’s right, we are only fertile for about 6 days of each menstrual cycle! With these signs we are able to identify the fertile window with 99.4% accuracy.

The three main fertility signs are:

  • Basal body temperature (BBT): Your resting temperature, which rises after ovulation.
  • Cervical mucus: The texture and appearance of mucus changes throughout the cycle, becoming clear, lubricative and stretchy around ovulation.
  • Cervical position: The cervix shifts in height, softness, tilt, and openness throughout the cycle.

By observing these signs daily and recording them, you begin to see patterns in your cycle. Over time, this gives you clarity about when ovulation happens and what’s happening hormonally in your body.

HEADS UP: Unlike the “rhythm method” (which guesses ovulation dates based on average cycle length), fertility awareness methods (FAMs) are scientific and personalized. They’re based on real-time fertility sign observations, not calendar predictions.

That is to say, when you hear someone say (even a doctor!) that every woman ovulates on day 14 of their cycle, this couldn’t be farther from the truth!

People turn to fertility awareness for many reasons, and often, more than one:

  • Some use it as a natural form of family planning (avoiding pregnancy)
  • Others use it to better identify their fertile window and increase their chances of conception.
  • Your menstrual cycle is like your Fifth Vital Sign — they can reveal issues such as thyroid imbalances, PCOS, luteal phase deficiencies, and much. more.
  • Many people use cycle charting to advocate for themselves in healthcare. Your chart can be used as a starting point for your diagnosis, and in many cases, a proof of when conception happened in order to calculate the precise gestation date of your baby.
  • Tracking fertility is a way of saying, “I want to know my body better.” By charting your cycles, you will know what is normal for you (your baseline), and you will be able to quickly identify when something is not ok.
  • It fosters curiosity, confidence, and a sense of safety in one’s own rhythms. You can use your cycles to plan your life and activities better, and learn what impacts your hormonal levels the most (sleep, diet, stres, etc.) and in what ways.
  • Some choose fertility awareness because they prefer not to use hormonal contraceptives. Even though long-term studies are still scant, there is enough scientific evidence on the side-effects of contraceptive drugs.
  • Many are prescribed hormonal contraceptives to suppress the body’s innate hormonal production and reduce symptoms (irregular periods, PCOS, acne, etc.). However, this does not treat the cause of the symptoms – rather, it only masks the underlying problem and the symptoms resume once the patient stops taking them.

Learning fertility awareness isn’t about memorizing facts; it’s about building a daily habit of observation. It’s a fine-tuning with your body, and a curiosity-lead learning process!

Here’s what it often looks like:

  • Each day, check for signs of cervical mucus and note how it feels and looks.
  • Take your basal body temperature each morning before getting out of bed.
  • Check your cervical position in the shower or before going to bed.

Write observations in a paper chart or use an app (I love the Read Your Body App!).

  • With practice, you’ll see your cycle’s pattern: menstruation → follicular phase → ovulation → luteal phase.
  • You’ll learn which days are fertile and which are not.
  • You’ll see how your hormone balance is translated into your observations (mucus quality, temperature ranges, etc.).
  • If avoiding pregnancy, you abstain or use barrier methods on fertile days.
  • If trying to conceive, you time intercourse during fertile days.
  • If tracking for health, you use your chart as data to share with your health practitioner.

Because fertility awareness is often confused with less reliable methods, let’s clear up some myths:

Absolutely not! The rhythm method relies on averages; fertility awareness uses real-time data. We never assume a woman ovulates on day 14 of the cycle, and we can’t even assume that every bleed is a true menstrual bleed. When women are trying to conceive, we will never tell her to try on the 14th day of her cycle. Instead, we will observe her mucus patterns and instruct her to try on her peak mucus days. If a woman is trying to avoid pregnancy, we will never tell her that she is only fertile on or “around” the 14th day of her cycle. We have very specific and scientifically tested rules on when to open and close your fertile window. That is what makes the method I teach 99.4% effective for contraception.

On the contrary! If you’re struggling with irregular cycles, charting your cycles with the fertility awareness method will allow you to see exactly what’s happening in your body. Are you ovulating? When? Are you producing enough mucus? Are your temperatures within the range of normal, or should we screen for thyroid issues? Apart from helping you to identify why your cycles are irregular, you’ll be able to take action to improve them, and see the progress in your following charts after you implement new habits and treatments.

There are many methods within the term “Fertility Awareness”. Some methods only use one fertility sign, such as mucus only methods. Others use ovulation strips and mucus as the main fertility signs. The method I teach (FAMM, as taught by Lisa Hendrickson -Jack), is 99.4% effective when used perfectly. It’s not only very effective for contraception, but it’s also used by women who have been trying to conceive, as it can help them to improve their reproductive health, egg quality, and time intercourse with their fertile window.

Fertility awareness is for anyone who wants body literacy, empowerment, and optimal health, whether or not pregnancy is ever a goal. However, as stated above, it can significantly increase your chances of conception. And don’t forget that we are only pregnant for a 9 month period. It can be immediately be used as contraception after giving birth. Learning the fertility awareness method can be a useful tool for whatever phase the life you might be in. 

  • Knowledge is empowering. Knowing yourself, keeping track of your moods, your energy levels, your fertility signs, and your body in general is the most powerful tool you have to maintain health and vitality. You’ll understand your cycle more deeply than most people (even doctors!) ever do. As I always say, tracking and charting your cycles is like having a monthly health report. Learning to read and interpret them is your key to a truly embodied life.
  • No side effects. Your body is doing what it was designed to do, and you are supporting it with the right nourishment and lifestyle it needs to thrive! There are countless benefits to naturally cycling, ovulating regularly, and having healthy hormonal balance. Suppressing your body’s basic biological functions such as with hormonal contraceptives might have its benefits too, and can be the best solution for certain cases. However, this has been shown to negatively impact overall health in the long term, causing several life-threatening diseases. The only side effects that fertility awareness might have is becoming obsessed with charting your fertility signs and never being able to shut up about it (guilty!).
  • Holistic view of the body. Fertility awareness is nor only useful for identifying your fertile window for conception or contraception. Many women chart their habits, nutrition intake, mood, energy legels, and secondary fertile signs alongside the main fertility awareness variables. It becomes much easier to recognize the patterns throughout the different phases of the cycle, and to make the appropriate arrangements to better navigate them. For example, when you notice your peak motivation and energy is around ovulation, you can plan to have your workout sessions, meetings, or appointments during those days. Or when you know around what cycle day you expect to have an energy drop, you can choose lighter activities. If you’re working on improving nutrition, you can see in real time how that affects your mood, your temperatures, mucus production, cycle length, menstrual pain and flow. It’s not just about fertility, it’s about your sense of self in all levels!
  • Stress-free—you are in control. Women who have been using the method consistently quickly realize that monitoring your fertile window gives us great peace of mind. While relying on other contraceptive methods or fertility treatments can feel less stressful for others, it’s important to remember that these methods and procedures also often fail. As a user-dependent method, you are in control of when you have intercourse, and you will be able to take quick action in case a barrier method fails or if you think you might have made a mistake. That means using emergency contraception in time, or choosing to terminate an unwanted pregnancy as soon as possible. In terms of fertility treatments, these are often very invasive and costly procedures, and often need to be repeated before achieving pregnancy. Sometimes the problem can be solved simply with improving egg and sperm quality, tracking ovulation and cervical mucus for better timing intercourse, or solving some underlying health issue that is undermining fertility. If you want to feel in control of your choices, of your body, and your health, this is the right method for you.

Every method has limitations, and fertility awareness is no exception. It might not be the best method for you in this specific time of your life. It might not be

  • Proper training. It can feel overwhelming to learn on your own, and it is highly recommended to find an instructor that can teach you the method step-by-step and in detail.
  • Learning curve. It takes time (often 3–6 months) to fully understand the reproductive anatomy and physiology, create the habit of observing and charting your cycles, and feel confident applying the correct rules of the method.
  • Consistency. You need to observe and record daily. It is a user-based method, which means that the success of the method relies on the user itself. It doesn’t mean that missing a few days of data in your chart is the end of it all – it just means your effectiveness will decrease.
  • External factors. Illness, stress, or travel can affect your cycles, but you will know how to work around them once you understand how your body works.
  • Proper training. It can feel overwhelming to learn on your own, and it is highly recommended to find an instructor that can teach you the method step-by-step and in detail. 

Choosing whether to practice fertility awareness depends on your goals and lifestyle. Ask yourself:

  • Am I willing to track daily for at least a few months to learn?
  • Do I want a hormone-free option for family planning?
  • Am I curious about my body and open to learning?
  • Would I value having more information about my health and reproductive issues?

If you answered “yes” to any of these, fertility awareness might be worth exploring.

If you feel curious, know that there are many fertility awareness based methods. Some use only cervical mucus as a fertility marker, some use hormone testing (i.e. ovulation strips), some use calendar calculations. Here is what I recommend checking out for now:

Fertility awareness is more than a method — it’s a mindset of curiosity, empowerment, and self-care. It allows you to step into partnership with your body rather than trying to mask symptoms or completely ignore it.

Whether your goal is pregnancy prevention, conception, or simply self-understanding, fertility awareness offers both knowledge and self-compassion. It’s a way of saying: “My body is worth knowing. My cycle is worth understanding. And I am worthy of empowerment and care.”
